Complete the image order and permission form and return to us. Payment is required when you place your order. Please make checks payable to the Utah State Historical Society.

Beginning July 1, 2011 State History will implement new photo reproduction prices and use charge fees.
Rush orders are no longer accepted. Please allow 7 to 10 working days for completion of digital image orders. Please allow approximately two weeks for completion of photographic print orders.
